I fly the B-1. The ladder is separate from the door. There is a door on either side of the ladder, one closed from the cockpit, one closed when the ladder comes up. The ladder is capable of being jettisoned if the ejection seat fails...we carry one spare chute, and you can bailout through the entry door.

I saw a B-1 on static display. The entrance to the cockpit is underneath the aircraft behind the front landing gear. A door swings open and a ladder is extended. (I assume that the ladder is mechanically part of the door, but it could be extended by the crew.)

I just get a really gay vibe from him now that I didn't pick up on when I was a kid. But the 80s were very homophobic times.
